A common misconception about the DOT SAP evaluation process is that it applies only to commercial truck drivers. While trucking is one of the most visible industries regulated by the Department of Transportation, SAP evaluations are required for many other safety-sensitive roles across the transportation sector.

DOT drug and alcohol regulations apply to employees performing safety-sensitive functions under agencies such as the Federal Motor Carrier Safety Administration (FMCSA), Federal Aviation Administration (FAA), Federal Railroad Administration (FRA), Federal Transit Administration (FTA), Pipeline and Hazardous Materials Safety Administration (PHMSA), and the United States Coast Guard (USCG). Any safety-sensitive employee covered by these agencies who violates DOT drug or alcohol testing rules must complete a SAP evaluation.

A DOT SAP evaluation is mandatory after a verified positive drug test, an alcohol test above the allowable limit, or a refusal to test. Once a violation occurs, the employee must be immediately removed from safety-sensitive duties and referred to a DOT-qualified Substance Abuse Professional. This requirement applies regardless of job title, employer size, or industry segment.

The SAP evaluation assesses the circumstances of the violation and determines the appropriate education or treatment needed to reduce risk and protect public safety. Drivers, pilots, transit operators, railroad workers, pipeline workers, and maritime employees may all be subject to the SAP process depending on their role.

After completing the SAP’s prescribed education or treatment, the employee must undergo a follow-up SAP evaluation. Only after receiving written authorization from the SAP can the employee take a DOT return-to-duty test. A negative test result is required before resuming safety-sensitive functions.

SAP evaluations also include follow-up testing plans that may last up to five years, reinforcing ongoing compliance and accountability. Employers are legally required to implement these plans in addition to standard random testing programs.
