ROLE OF THORACIC SPINE MANIPULATION AND SCAPULAR STABILIZATION IN SUBACROMIAL SHOULDER IMPINGEMENT SYNDROME

ABSTRACT
Subacromial impingement syndrome can be defined as inflammation and irritation when soft tissues of the shoulder, such as rotator cuff tendons and bursa, are compressed within the narrow space between the shoulder blade and the top of the arm bone. It brings acute pain, a problem in movement and affects the ability of a person to perform daily activities seamlessly. Application of thoracic spine manipulation and scapular stabilisation is a possible way to reduce pain, improve joint function, and recover shoulder posture to enhance the medical condition of a patient who has been suffering from subacromial shoulder impingement syndrome. Therefore, this article discussed the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome. A secondary qualitative approach has been used along with a thematic analysis process to bring forth effective findings within the article. In addition, the research paper of Manas Kumar 1 and Dr Annu Jain, as “randomised clinical trial on the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome”, has been used as a key secondary data source to perform this article. As per the findings of this research, the combined implication of thoracic spine manipulation and scapular stabilisation mostly helps to deal with patients with subacromial shoulder impingement syndrome compared to traditional physical therapies.

Introduction
Subacromial impingement syndrome refers to inflammation and irritation of the rotator cuff tendons as they pass through the subacromial space. SSIS causes pain, weakness, and reduced range of motion within the shoulder. The help of chiropractic-based manual adjustment, thoracic spine manipulation and scapular stabilisation can help to deal with subacromial shoulder impingement syndrome effectively. The subacromial impingement syndrome (SIS) represents a common cause of disability in approximately 74% of patients with Shoulder Pain (SP). Furthermore, Subacromial shoulder impingement syndrome (SSIS) represents one of the most prevalent disorders affecting the shoulder complex, causing 44-65% of all shoulder complaints in clinical settings. Therefore, this article focuses on discussing the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome.

Discussion on shoulder impingement syndrome
Shoulder impingement syndrome (SSIS) is a painful situation where soft tissues of the shoulder, such as rotator cuff tendons and bursa, are compressed within the narrow space between the shoulder blade and the top of the arm bone. SSIS can be defined as mechanical compression of the subacromial tissues against the anterior undersurface of the acromion and coracoacromial ligament in time of arm elevation . It causes acute shoulder pain that affects quality of life. In other words, shoulder impingement syndrome is a painful condition due to structural narrowing of the subacromial space .
Analyse the concept of thoracic spine manipulation and scapular stabilisation
Thoracic spine manipulation (TSM) is a therapy that involves a high-velocity, low-amplitude thrust to a segment of the thoracic spine to reduce pain and improve mobility. The process of TSM involves skilled, passive, high-velocity, low-amplitude movement directed at the joints and soft tissues of the thoracic spine . On the other hand, scapular stabilisation refers to the process of using the muscles surrounding the shoulder blade to maintain its proper position on the ribcage during arm movements, providing a stable base for the shoulder. Scapular stabilisation is a stretching exercise that is used to improve scapular movement to reduce pain, improve shoulder function effectively .
Role of thoracic spine manipulation and scapular stabilisation to deal with SSIS
The application of the thoracic spine manipulation and scapular stabilisation supports in reducing shoulder pain, improving joint mobility and enhancing shoulder posture. Methodology
This article focused on using secondary qualitative data to bring forth effective findings on the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome. Secondary data collection process helps to gather relevant information from previously published primary research output from different authentic and peer-reviewed articles and journals available online. Furthermore, qualitative data are the textual sources of data that help to develop an effective understanding of a research topic. This particular research needs a larger source of textual qualitative responses to understand the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome. Therefore, a secondary qualitative data collection method has been used to perform this study effectively. Furthermore, thematic analysis has been used to interpret the secondary qualitative data to develop an effective understanding of the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome.

Results and findings
Theme 1: Immediate Efficacy of Thoracic Manipulation on Managing SSIS
The immediate result of the thoracic manipulation helps to enhance neurophysiological mechanisms as well as provide pain relief. The application of the Thoracic Manipulation helps to improve the medical condition of a patient with Subacromial Shoulder Impingement Syndrome by having an immediate effect on joint mobilisation. Application of the Thoracic Manipulation provides key assistance towards reducing pain, increasing range of motion, and enhancing scapular kinematics, facilitating neurophysiological mechanisms as an immediate effect. It mostly helps to deal with Subacromial Shoulder Impingement Syndrome.
Theme 2: Long-Term Functional Outcomes and Scapular Stabilisation to deal with SSIS
The long-term impact of Scapular Stabilisation helps to enhance the intervention success for treating patients with Subacromial Shoulder Impingement Syndrome. Long-term functional outcome Scapular Stabilisation includes reducing shoulder pain, improving shoulder range of motion, enhancing functional capacity to perform daily activities and correcting abnormal scapular movement patterns. These processes provide key assistance towards improving the medical condition of patients who have been facing Subacromial Shoulder Impingement Syndrome.
Theme 3: Clinical trial on the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome
A clinical trial on the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome has been performed by Manas Kumar and Dr Annu Jain, which helps to develop effective findings within this article. The mentioned article by Manas Kumar and Dr Annu Jain has been performed by using a primary quantitative process where statistical data has been interpreted by using SPSS software. As per the randomised clinical trial, it has been found that a combined approach of thoracic spine manipulation and scapular stabilisation exercises results in superior improvements in pain, function, range of motion, and scapular kinematics compared to standard physical therapy. It significantly assists in improving the condition of a patient and offers effective pain relief for patients with subacromial shoulder impingement syndrome.
Conclusion
This article focuses on discussing the role of thoracic spine manipulation and scapular stabilisation in subacromial shoulder impingement syndrome. A secondary qualitative approach has been used to bring forth effective findings within this article. As per the findings, the application of horacic spine manipulation and scapular stabilisation improves shoulder movement, posture, and joint functions and reduces shoulder pain. In conclusion, a combined implication of the thoracic spine manipulation and scapular stabilisation significantly helps in dealing with patients with subacromial shoulder impingement syndrome and achieves more effective results compared to traditional physical therapies. Therefore, taking expert chiropractic assistance to incorporate thoracic spine manipulation and scapular stabilisation helps patients to recover from subacromial shoulder impingement syndrome and enhance comfort in performing daily livelihood activities successfully.
