Is Your Lab's Denudation Protocol Actually Optimised? Most Aren't

Ask most IVF lab directors what their denudation protocol is and you'll get a confident answer. Ask them when it was last audited against outcomes data and you'll often get a pause.

 
Denudation the removal of cumulus and corona radiata cells from a mature oocyte prior to ICSI is performed in every ICSI cycle. It's one of the most repeated procedures in reproductive medicine. It's also one of the least formally governed by written, evidence-based protocols in most laboratories.

That's beginning to change. Here's what the conversation looks like in 2025.

The core concern is the meiotic spindle. This structure, invisible under conventional light microscopy, governs chromosome segregation at fertilisation. It's sensitive to temperature change, chemical exposure, and — critically mechanical disruption. The pipetting technique used during denudation applies direct mechanical force to the oocyte, and the evidence suggests that the magnitude and nature of that force matters more than most labs have historically assumed.

Studies using Polscope imaging have demonstrated measurable spindle displacement following aggressive mechanical stripping. Labs that have reduced pipette passes, shortened hyaluronidase exposure, and standardised their sizing sequences have in several cases reported improved fertilisation rates — without changing any other variable.

The practical implication is straightforward: if your lab hasn't reviewed its denudation protocol in the past two years, it's worth doing. Review hyaluronidase exposure time. Look at the pipette sizing sequence your embryologists are using. Check whether there's variation between individuals. Cross-reference against fertilisation and degeneration KPIs.

It won't take long. And it might move your numbers more than the next media trial.
