IMS and SUM Hospital has successfully performed its first Transcatheter Aortic Valve Replacement (TAVR), treating a 71-year-old woman who had been admitted in acute decompensated heart failure with cardiogenic shock. The procedure was performed by interventional cardiologist Dr. Gyana Ranjan Nayak. The patient was up and walking within two days and has since been discharged home.
The patient was admitted to the hospital's cardiac intensive care unit (ICCU) and diagnosed with severe calcific aortic stenosis, a narrowing of the aortic valve caused by calcium build-up. Her assessment showed a peak gradient of 140 mmHg, a mean gradient of 89 mmHg and a left ventricular ejection fraction (LVEF) of 30 percent, indicating a severely weakened heart. Her condition was too unstable for her to be shifted out of the ICCU.
After discussing the case with the patient, the cardiology team decided to proceed with TAVR, a catheter-based alternative to open-heart valve surgery. The procedure was carried out through the percutaneous femoral artery route. It was considered high-risk because of the anatomical challenges involved.
The patient recovered well and was discharged home within days of the procedure.
"Cases like this come with real challenges in cost and logistics, and they are never the work of one person alone," said Dr. Gyana Ranjan Nayak. "With this kind of determination, interdepartmental teamwork and support from our management, I am confident we can continue taking on complex cases here, so our patients do not have to travel to other states for quality cardiac care."
The case was supported by the hospital's management and by the Cardiology, Cardiothoracic and Vascular Surgery (CTVS) and Cardiac Anesthesia departments. The Radiology department carried out the detailed pre-procedure assessment. Dr. Nayak recently completed a TAVR fellowship at GOKVI.
