Precision medicine has built its reputation on a paradox it has never fully resolved: for every patient who experiences a dramatic, sometimes complete, response to a targeted therapy, several more see little or no benefit at all — and the stakes could not be higher. Patients are frequently left in a diagnostic dead end: genomic testing tells them what mutation they carry but says nothing about whether the drug matched to that mutation will actually work in their body, or why it might fail. The field has spent decades defining which drug to match to which mutation, while largely ignoring the deeper question of which patients simply won't respond, and what could be done to convert them into responders instead.

The answer could lie in PGA (Patient-derived Gene expression-informed Anticancer drug efficacy) drug mapping or drug-finding technology — an approach that look beyond static DNA sequence and instead capture how a patient's genes are being hyperactively expressed, offering a functional, real-time signal of drug response that a mutation panel alone cannot provide.

This shift matters because DNA is necessary but not sufficient: two patients with the identical driver mutation can have wildly different treatment outcomes, and the missing variable often lives in de-regulated gene expression and tumor biology rather than in the genome itself.
