The numbers alone tell a damning story: BayCrest holds a 1-out-of-5-star rating from Medicare — the lowest possible — has accumulated 59 health citations against a California average of 15.7 and a national average of 9.6, and has been fined over $100,000 in the last three years, including three major fines in late 2025 alone: $68,085 in November, $19,115 in October, and $24,845 in August. The facility is designated a Special Focus Facility Candidate — a federal watch list for the country's most persistently failing nursing homes. Regal Medical Group authorized Todd's placement here anyway.

State inspections have documented residents left in soiled diapers for extended periods, patients going weeks without basic oral care, broken windows, a pervasive smell of urine, unnecessary physical restraints, and systemic failures to maintain residents' mobility and range of motion. These are not allegations. These are substantiated findings from government inspectors.

The Kitchen: A Daily Clinical Failure

Todd Graham is a diabetic. That is not a background detail — it is a life-or-death medical fact that every staff member, every cook, and every administrator at BayCrest Care Center was fully aware of from the moment he arrived. Diabetes demands precise, physician-ordered nutritional management. Deviation is not inconvenient — it is dangerous. It can cause blood sugar crashes, organ damage, infection, limb loss, and death. BayCrest's kitchen has consistently, repeatedly, and inexcusably failed to meet that standard. Todd has been served low-protein meals that directly violate his doctor's dietary orders. He has been given food that a physician explicitly prohibited. The meals described by his family as "disgusting" are not merely unappetizing — they are medically contraindicated for a diabetic stroke survivor managing hypertension.

This is not a cafeteria complaint. This is a clinical failure being committed three times a day, every day, by a facility that is being paid to keep this man alive. And the consequences extend beyond blood sugar. Stroke recovery depends on neurological rehabilitation — and neurological rehabilitation depends on muscle rebuilding. Muscle rebuilding requires adequate dietary protein. Without sufficient protein intake, muscle fiber cannot repair, cannot grow, and cannot strengthen — no matter how many hours of physical therapy are performed. BayCrest has already provided Todd with only one month of therapy across a five-month stay. But here is the compounding tragedy: even if BayCrest had provided full, consistent, daily physical therapy, it would have been largely wasted. You cannot build muscle on a low-protein, nutritionally deficient diet. The kitchen's failures have not just endangered Todd's health in isolation — they have actively undermined any possibility of meaningful recovery. His family has been forced to personally purchase appropriate diabetic meals and fund private therapy out of their own pockets simply because BayCrest cannot or will not fulfill its most basic obligation. A kitchen that cannot feed a diabetic stroke survivor according to his physician's orders is not a kitchen. It is a liability. And in Todd Graham's case, it has been a slow, daily, institutional assault on his health and his future.

A welfare check requested from the Torrance Police Department yielded no formal report, as officers cited a lack of jurisdiction over licensed care facilities.

BayCrest operates under Genesis HealthCare, currently in Chapter 11 bankruptcy. Regal Medical Group authorized this placement. Genesis HealthCare operates this facility. Together, they bear direct responsibility for what Todd Graham has endured.
