The expanded care service provides fast-track home readiness assessments, mobility equipment setup, and dedicated reablement packages within 72 hours of discharge notification. Care staff deliver personalized assistance with personal hygiene, medication administration according to prescribed records, and tailored meal preparation designed around specific dietary requirements.

In addition to supporting post-hospital recovery, the agency collaborates closely with Haringey Social Services to manage council-funded care packages, Direct Payments, and NHS Continuing Healthcare arrangements.
